A service plan will be completed for each client. It will be prepared by the Registered Nurse after initial in-home assessment has been completed. The service plan document must have been completed by the time the second visit takes place, or within 7 business days of the first/initial visit, at the latest. The client and/or his representative will be allowed to take an active part in the creation and revision of the service plan. The service plan document will include the following items: • Specific functions. • Specific services the agency has agreed to provide. • The duration, frequency, scheduled time of the services to be delivered/provided to the client. • A written description/directive of how each service rendered will be delivered. • Limitations of the client. • Objectives of services rendered. • Statements of desired goals. • Discharge plans. The service agreement will be reviewed every 92 days or 122 days for services or earlier according to any changes in client needs or service provision. Where applicable, the service agreement will include pertinent diagnosis, diet and nutritional needs, current medications and treatments, equipment needs, and any other related information.
